His wound must be thoroughly cleaned with plain water and soap soon after the incident. He should receive rabies vaccine as soon as possible. If there was bleeding from wound he will need rabies immunoglobulins also visit the pediatrician for complete analysis of the scratches and further course of treatment

your child could have UTI ( urinary tract infection ) needs evaluation, you must reach out to either a pediatrician or Pediatric urologist then to them to get evaluated as there could be because of various reasons and needs evaluation by expert, once evaluated then can be treated appropriately
