Could be any other rash,like Atopic dermatitis.
For sweat rash
Do leuk warm bath twice a day.
Wear loose cotton of clothes

Nail infection due to fungus is Called as Onychomycosis.
Use oral Antifungals pulse therapy

Have you used any local creams on own or purchased from medical shop.
Stop creams if using any not prescribed by dermatologist.
